General Information
Access Cards & Photo IDs
- All requests for photo identification cards (IDs) and/or ADT Proximity access cards for 79 Elm Street, including their replacements, should be made directly to Sandra Nicholson of the Bureau of Central Services via email.
- Employee photo IDs are required to be visible at all times when employees are at 79 Elm Street. Employees without IDs must sign in with Security for the day and receive a temporary visitor's card.
- All employees with building access are reminded to use the employee entrance on 79 Elm Street and not the main lobby door. This will allow Security to focus their attention on the visitors entering the building. Employees who have assigned parking in the building garages may use the entrances located in their assigned garage. Proximity cards must not be loaned to other employees. It is agency policy that all employees use their own access card when entering the employee entrances.
- All ADT Proximity Cards are automatically set to “Auto Disable” if you do not use your access card within 30 days of each month. If your card should “Auto Disable”, please email Sandra Nicholson for instructions to reset your proximity card.
- If a Field or Seasonal employee requires building access, a written request (including justification) must be submitted by his/her director to Sandra Nicholson of the Bureau of Central Services.
- If an individual who is not a DEEP employee (i.e., Intern, Volunteer, Contractor, "Temp", etc.) requires building access, a Request for Access Card (Non-DEP Employee) form must be completed, signed and returned to the Bureau Chief of Central Services for approval. If the request is for non-standard building access levels and/or days/hours, the appropriate justification must be attached to and submitted along with the completed form.
- If a card is lost, please notify Sandra Nicholson immediately via email or at 860-424-3606. Please note that the Department of Administrative Services charges $10.00 for lost or stolen proximity cards.
- Upon termination of employment or services rendered at the DEEP, the individual’s ID and ADT proximity access card(s) must be submitted to Sandra Nicholson with a signed Return-Transfer of State Property by Exiting Employee form. The “Return” form must also be signed by the employees supervisor.
- All access levels are pre-determined by your own bureau and any questions should be directed to your bureau's business office.
